parallax forms

Looking for a parallax expert to help me create a scrolling contact form that will save information to a mysql db. I already found a parallax page that partially does what (i.e. fade in and out of text) The background color/image will remain the same while the scrolling text will fade in and out asking the user to enter first name, last name, street, etc Similar effect to what is available via [login to view URL] MAX BID: $20
