We want this project been done very urgent. Your cooperation in this would be very appreciated and awarded correspondingly. Please let me know your price and when you can have done this. We want to have a scription form in which we can upload the : -comment -name (obliged) -email (obliged and only one submission) -url (not obliged with url check) -foto (check filesize:100kb and fileformat:jpg/gif and check + proportion 2:3) -2 checkboxes to agree upon conditions -predefined background as in the attachment After submission the user has to validate the submission and both the user and us receive a confirmation email. The email text can be managed in the config file. The data will also be registrated in a MySQL database. Also a list of IP-blockings and emailadress can be maintained in the config file. All details are in attached file.
