Very Simple PHP Contact Form Management

Hello, I have a little small PHP form management project. Project is developed on PHP-MySQL and its %75 completed. Its missing part is simple small administration screen. I'm also a developer but don't have a time to complete.. If you are expert in PHP you will complete it not more than 1-2 hours (believe me its already complete) Please check the uploaded files Its a contact form. What i need on the admin area is: - Administrator can select which fields are required - Administrator can manage the questions (which will be shown a<s checkboxes) - View the contact form results (from db. it saves to the db) - Manage the email which recieve the informations (also its written on the database) when the form is submitted. Thats all. No authentication required. Its simple as A-B-C Budget is 30$ If you can complete more quickly maybe i'll give a bonus. Please ask any questions that you don't understand..
