Typesetting a book

I want to convert a book of 192 pages (Word file) to E-book and POD. The online PDF (E-book) should be be 4 color and without blank pages. The print PDF (POD) should be single color and every new chapter should start on right page (Recto opener). The E-book should also be compatible with Kindle, Sony E-reader and i-pads. Table of contents should be prepared. Index entries will be marked [login to view URL] needs to be generated. Figures may contain line arts and/or half tones. Cover design will be provided and should be re-sized according to the spine width. This will be mostly a paperback printing. This is a sample project majorly for cost estimation. Clean and transparent bids are expected. No hidden charges should be claimed, post or in the process of production. The books usually go to printers directly. Quality should be of that high level. Indian freelancers preferred.
