Layout of a one page press kit sheet

I need a single 8 1/2 x 11 page press kit sheet created. The attached Media Sheet PDF file page 1 contains the sample that we would like to replicate. Pages 2-4 contains the text to be used to replace the text in the sample. Pages 5-6 are existing sheets in our press kit to be used for styling, etc. This is a quick turn around project. The bidder should expect to produce a sample and several rounds of tweaks once the sample is reviewed. Samples to be provided in PDF format, final work to be provided in PFD format as well as original high res photo shop files. Additionally some graphics files are attached as well as an additional doc to use in the body text.
