Hi, I am a VFX artist. This is not a 3d modelling job but a painful frame by frame rotoscopy which could burn a hole in your pocket. Even then the result may be inconsistent because there is no clean plate and lighting information on the face is varying by lots of degrees. It would be easier if you could reshoot the scene and place masks on eyes and lips so that it becomes easier in the compositing stage and bring down the cost.
