Service Request / Open Ticket Program

I am looking to have a system that is web based. The system will do the following: A job request form is filled out, and once the form is submitted, an email notification will go out to my service guys. The form will be simple with name, address, number, and request type (a section to add notes on the nature of the job request) The job request then will go into a "open" status. Once one of the service guys accepts the job, the job then goes into a "working" status. When the service guy is done with the job, he changes status to "closed". The list of jobs must be able to be edited, and once closed it should go into a "closed" jobs section, that we can access. Would be nice to have a button for directions to the address for the customer linked to google or mapquest(Not necessary).
