Personization Website - Variable Data

We need a page with a form consisting of first name, last name fields only (no email address) and a submit button. On submitting the form visitors will be directed to a separate sales page. Entry to this sales page must require first and last name being entered. The first and last name entered by the visitor will be captured as variable data and displayed throughout the sales page within the text and on images and graphics. For example - text might read "Dear <firstname> " - whilst a graphic on the page might be a dummy business card on which the persons first and last name are also displayed - or a postcard with their name on the front. There are multiple images throughout the page. Pages need to work on mobile as well as standard web browsers.
