Assess and Score Questionnaire and then save results in database and email user one of three possible responses

I require a tool/plugin/software to do the following on my website: 1. Ask a series of questions mainly multiple choice. 2. Score the responses and then assess eligibility at the end of the assessment quiz (this is to assess for eligibility to immigrate to a country) 3. Store the responses somewhere and email the responses to my email account. 4. Then depending on the score send an email on three different possibilities: a. for those successful, b. different email for those not successful, c. response for inconclusive responses. This can be achieved either by modifying an existing assessment Plugin for Wordpress called Survey and Quiz Tools that has all functions except the email function. Responses should be stored in a database for easy access. Questions and system of scoring should be easily amendable without the requirement of programming. In other words I should be able to add or delete questions. Or this can done using existing quiz survey sites that can be programmed or any other inexpensive way of achieving this. Bidders should all explain how they plan on achieving this.
