Need SIMPLE website with mysql database

Hello, I'm looking for a skilled web programmer/designer that can help me with a project. I'm looking for someone who will be able to code and design an easy website where truck drivers who will have access to the site, will be able to write down their informations in 6 or 7 tabs/fields. The site will have to be designed with a responsive design so it will be nicely seen on a desktop computer, mobile phone or tablet. It does sound a bit strange now, but its very easy and not complicated. A couple of main point for the project: 1. The site will be private. Only accessible with a username and password. - Each user will have its own login - Administrator will be able to add/edit the new users - Each user will have profile details (Name, email and phone number) 2. Once logged in the user will be able to add a new field to the database. For example the user which is a truck driver will be able to write down, his location and date of the driver (date tab field, city and zip code field), what kind of load he has (truck weight, truck length, what kind of track he is driving, etc). There will be no more then 5 or 6 fields to enter. - Each field will have to be added or else the user will not be able to add the information 3. A database of all the fields and users should be made. Mysql I assume? I would like to have the sorting option for 1 or 2 fields. For example I would like to sort all of the informations by 'country' 4. Each user will be able to edit or delete his own fields. Just his own! 5. Each field will have an expiration time, every field will automatically get deleted after a day or two This are the main point of this project. This will probably be done in PHP and mysql. I have attached 2 screenshots from a site, I would like to have the same concept. The first one is showing how the input fields should look like. The second one is showing the main area with all the inputs, where you can view and sort the inputs. Just so you know, I will not need that many input areas as shown on the screen, I will only need 5 or 6. And the sorting option will be only be for two fields.
