Data collection web page

Webpage with content management system. The purpose of the site will be collecting the data and documents needed for tax declaration. Two parts: First part: the page as the end-user sees; this page will be in Dutch! * SIGN IN PAGE: The user has to create an user-account (user name and password), fill in the personal details (name, address, phone, bankaccount, emailaddress, social security number, date of birth) and accept the user agreement. * LOG IN PAGE: the user logs in to his account * QUESTION PAGES: If the user logs in he can fill in the needed information: ***** Part A consists of personal details (name husband or wife, social security number, date of birth , names and dates of birth of children). ***** Part B consists of several questions, that has to be answered with Yes or No. At the end of these questions a list appears with for each question that is answered with yes an edit field or upload option. The upload option points to a file on the computer of the user. During filling in these question the user has to be able to save the data. Security is very important here. Other persons must not be able to view or access this data. If the user is ready he can send all the answers, data and uploaded documents to me. Second part: the page the manager of the pages sees; this part can be in English. The manager must be able to modify the accounts of the users. The manager must be able to specify the questions of part B in the first part and specify whether this question requires an edit field or an upload possibility. Payment schedule: - 10% at start - 30% when first fully functional version is shown - 40% at final delivery - 20% only when delivered before deadline Other payment schedules are not accepted!
