Inserting custom form in Virtuemart checkout page

Hello! I have Joomla 2.5 site with Virtuemart 2 installed. I need someone to insert 2 custom forms on the checkout page. We sell some travel offers and we need some information about our customers (e.g. names, passport number e.g.) So I need an ajax cart where the shopper will choose how many people will travel, and after that some required fields that he has to fill in for each one that will travel with him. Another one form on the checkout page will be something like poll - where I will ask them one question, and where they have to choose the answer, something like "Where did you find us?" Thanks!
