361811 Google Map Web Application

I have a CSV with a list of 1400 addresses. The data is stored in the following columns: [Name of Place][Street 1][Street 2][City][State][Zip][Phone] I need to have a google map created that has the 1400 addresses plotted on it, along with a mechanism to search by zip code for the closets location. When the user clicks on the icon marking the location the balloon will pop up with all of the information list above. Thank you for your help, and please let me know any questions you might have.
