Quick HTML/ASPX work(repost)

**REPOSTED:** **YOU MUST KNOW HTML AND ASP.NET to some level. I've attached all the documents required. ** Just need someone to take a design from the first image below, an existing ASPX page (the second image), and make them look pretty close to the same. The final output is the ASPX page with the new fields. Second requirement: Must modify source code included in [login to view URL] to search on individual fields not just all of them. That will be clear when looking at the attached [login to view URL] file. I need this in **4 hours** UPDATE: I've included a few more images to give details. [![][1]][2] This is the information that needs to be added to the following aspx page [![][3]][4] The image that has all the fields is what the final aspx should contain. However, the design itself should match the other two images. CSS/ASPX etc will all be provided.
