Write a simple handlebars.js template for an ODK Survey

I am implementing a survey using ODK Survey and designing the form in XLSXConverter. The form is designed in .xlsx format and then converted to a .json file using ODK's online tools. The form allows a templatePath (see [login to view URL]) to alter the style and layout of a particular screen. These templates must be created using handlebars.js. I need a single template designed for a single question/screen of my survey. Specifically, I need a template which allows me to manipulate the placement of response options on the screen. For instance, the question has four response options -- I would like to place them in "random" places around the screen. The [login to view URL] template needs to be commented and include enough information that I (with no knowledge of JavaScript) can make minor edits and adjustments as needed.
