Node backend and JQUERY for HTML contact form

We have a HTML/CSS contact form on our webpage. We need someone to add a simple Node endpoint to our backend to send the content of the form to our email. We also need Jquery added to our frontend that sends the content of the contact form to the node backend. Once the form is submitted, the form should be replaced by text that says "Thank You For Contacting Us. We will reply shortly."
