Front-End HTML Multiple Files Upload Form

I need front-end source code of a HTML form. It should simply include a button called "select files..." and another button called "upload". The "select files..." button enables user to attach multiple files a time. User can click "select files" multiple times. And if user clicks "select files..." again, more files will be added without affecting those files that are already attached. The front-end UI should also enable user to remove certain files that are already attached. It means that the form needs to print out attached files information dynamically and provide a "delete" button (maybe a "X") at the end of each file item. Please provide me with all front-end code (HTML/CSS/JavaScript/JQuery, etc.). No back-end solution needed. You can leave the action field of the form empty. It should have similar front-end functionality like [login to view URL] I only need the front-end working. It doesn't need to be that fancy.
