Build a Website

Hi, I want to make one html page that I can save on my desktop. This page should have 2 fields to input data for City Name and Country Name. It should have a button called Submit. When a user enters some data like below and hits Submit, it should call a web service and display result on the page without refreshing the page. I should be able to just open this page from my desktop and click on the button after inputting the data to send data via soap and get the soap response from the web service and process it to display on the page . I can only use html, javascript, jquery, ajax for this.
