Create Windows 7 style folder icon with logo

We have an application which creates a "special folder" within the user's My Documents folder. We would like it to have a special folder icon, like "Pictures", "Downloads", etc. The result of this project is a single Windows .ico file, which is based on the standard Windows 7 folder icon but has our logo "within" the pages of the folder. The .ico file must contain all the sizes and colour depths of the standard Windows icon, and be visually consistent with with the Windows folders at each size and colour depth. We will supply the logo as a high resolution graphic. A high quality, visually appealing, icon is required, with proper transparency and colours. For the avoidance of doubt, we only need the graphics created. No programming is required.
