PSD to HTML Slice 1 page

*Preference to HTML5 / Javascript developers* We are looking for an experienced PSD -> HTML5 designer as we have 1-2 similar jobs per week. I have a relatively simple PSD that needs to be converted into HTML5. The HTML must be valid in all major browsers (Firefox, Chrome, Safari, IE7+, etc). I'm ok with CSS frameworks (960, etc) if that is your preferred work flow. The 4 form fields must be HTML5 (thankyou message without sending user to new page) and send email via smtp. The main components should be fixed-width and centered, while the header should be flexible and fill the whole window. Attached is the PSD to be sliced. Please provide portfolio examples and be able to start immediately.
