Build me an APP and hand over - SIMPLE CRM -- 2

This Project is for simple Customer relationship management. App requirements: - must work on iOS, Android, Windows - When you open the app I want two options - AGAIN SIMPLE CLEAN DESIGN - Option 1 - Add New Contact -Option 2 - To-do list. -option 3 - view all contacts Upon clicking Option 1 - I want the following fields - nothing required and everything freeform First name Middle initial Last name Phone Number Email Preferred contact toggle Email/Phone Text ok? Y/N Toggle Next follow up date (make this a date picker) Notes Click save, and it stores that contact information in the app. Upon clicking To-do on the main screen I just want a list of the contacts where the follow up date equals today, but I want to be able to change the day in case I want to see workload on a different day. Upon clicking all contacts there should be a search function and a list of all contacts put into NorthStar. I want the app to be named something like Northstar CRM with a slogan of "CRM Simplified, follow NorthStar." I do not have the time or the desire to send endless messages back and forth. If you think you can take the information above and make something out of it, then you're who I am looking for. I want someone that will just take it and run with it. DONT DAZZLE ME I seriously want it to be the simplest looking and functioning thing out there. Lets get that far, and then You and I can work together on adding functionality over time for additional agreed upon prices.
