I would like to have a Website developed with a MS Access database back-end. The site will have three major components as follow: 1. Home Page with login 2. Web form that appears after login 3. Admin panel that Webmaster can login to, to assign username and password, access information, etc. Here are the major components of each area: Home Page: Homepage should have areas for graphics and text as well as a login area for username and password and a “forgot password?? feature that will email a forgotten password to the user. Web Form: The Web form should appear immediately after login. It will be a standardized form with about 20 main categories and any number of sub-categories under each main. Once a form has been submitted the following should happen: The form is saved to a database, using a designated field as the primary key. The form is saved as a MS Word file (using specific formatting) to a folder. A copy of the MS Word document is emailed to the Webmaster. Please Note: Only the fields that are selected or contain data should show in the MS Word document. And each document must follow a specific format regarding fonts, page layout, etc. Additionally, the Word doc should be named according to a specified field. Admin Panel: The Webmaster needs to be able to login to assign usernames and passwords. The user will not determine them. The admin panel should also allow the Webmaster to change this information at any time as well as the email address that the forms are sent to. Please include posting this site as part of your bid. ## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
