ASP / SQL - Check validations, Change look and feel, change a report form

I have an application already developed, that needs: 1. Field validation checking: - Email - Phone - ID Number - Dates 2. Improve the way the final step of the process prints a form. All the fields are in the database, we just need to make it look better. It has to be printed, so we don't want to see the "print" button on the output. 3. Change the look and feel of the application. Right now it has a basic CSS style sheet. I will give you the TOP include design and some other graphic elements to be included to improve the graphics interface. I will provide 1. Access to the application 2. Graphic Elements 3. Example of the way I want the form to look.
