Custom PDF Form

I need a form creating that will have 3 sections:- 1. lenders details 2. loan details 3. borrowers details The form will be used as follows using acrobat reader only (both the lender and the borrower will only be using acrobat reader) [login to view URL] lender will fill out the lenders details and the loan details. The lender could also fill in some of the borrowers details. 2. the lenders details must then be locked (read only) 3. The form must then be emailed to the borrower, using the borrowers email address on the form (if not blank) 4. The borrower must be able to fill out the borrower section of fields. 5. The borrower must then be able to print the document. 2 copies must print 1 without a watermark and 1 with a watermark "Customer Copy" 6. the borrower will then manually sign the document and manually post it back to the lender. The deadline for this is pretty tight and would like to get someone working on this today. Applicants must be contactable via skype and have good command of English.
