Ajax forms

Hello i need some buttons where when we click the button a small flash or Ajax or popup window opens asking enter your Telephone number . when they enter there telephone number they should send a script to my server which i got ready so what you have to do is just design the buttons and form in an attractive design and insert . for example you can check [login to view URL] for this kind of buttons .
