Create MySQL Database around HTML/CSS Templates

I require a database and web frontend to store information on servers and hosted accounts. We want to use MYSQL for the database with dashboard admin theme for the frontend. [login to view URL] The information stored in the database on servers will include the following fields server name, IP address, number of accounts on server and the maximum number of accounts permitted on that server. The information stored in the database about accounts will include the following fields server where the account is hosted, username, billing system ID, and quota. A secure administration login script is required which will be used by one user. The pages I require are as follows. 1. Login screen (we already have the designed login page but this will need to be coded) in to the login script. [login to view URL] of servers (this will be the index page) ID | Server | IP | Accounts | Max Accounts | Edit server When you click a server in this list, the accounts which are hosted on this server will be listed with the following fields. The database and web frontend will be hosted on a Linux based server(Centos 6)
