I have a 5-step excel form that needs to be converted to php/mysql online form. In this form user adds information selecting variables from drop-down boxes and numerical values about them. After selecting all the variables and inserting the appropriate values required in each step, user moves on. At the last step system produces 4 graphs and 2 "score" tables. Variables and values are connected through numerical equations that pass values and results to the next step. Current file as it is based on MS Excel and gives us the ability to make additions/changes in variables. In its on-line version we will need an extra admininstrator interface in which the admin (no registration forms needed) will be able to add extra fields to all the drop-down variables (titles or explanations of the variables) and one extra value regarding this variable's weight/value in the equations that it is used. Some nice ajax pop-up messages would be nice to warn user for missing fields. As I said there are 5 steps, but only 3 of them are functional, the rest are just welcome messages and at the final step user selects to either send the summary with his scores to himself through email or just complete the process. The 3 "core" steps include 7 areas with variables and on the 4th step user receives the summary with 2 tables and 4 graphs (2 column and 2 pie charts) . The excel file will be provided to the winning bidded.
