Web Application - Personal Plan Development

We are hiring a developer to create only the beginning of an web-based application. If this milestone is completed well, there will be more work [login to view URL] requirements are below. This is an interactive application that will ask a series of questions and develop a plan based on the answers. For completion of this project, please have this up and running on a development webserver I can access to test its functionality as described. Please also send a zip file of the entire source code. Login/account creation: Ask if the individual is a new user or a returning user. If returning user, provide login and password fields. If new user, collect name, email address, username, password. Password – require one number and at least 6 characters. Make sure user names are unique and that emails are unique. Send an email to activate the account. Email is editable by us. Generate a link that will activate the account. When the account is activated, allow login. Create Plan: Click on a button to create your plan. Create a “name” for your plan. Ask a series of questions. All questions will follow the same general format: 1. Ask the question 2. Allow me to enter text of any length, links, etc 3. Provide place for answers Question 1: How many people are you planning for? “Blah blah blah” text I enter later Answer: Choice of 1 to 100. Question 2: Provide name and age of each person you are planning for. “Blah blah blah” text I enter later For each person in #1, Name should be in one name field Age should be by age group:(up to 2, 3-5, 6-10, 11-17, 18-30, 31-40, 41-50, 51-64, 65+) Question 3:How many days are you planning for? “Blah blah blah” text I enter later Allow choice of from 3 to 365. Calculation: Multiply #1 x #3 and say: Goal: Plan for x people for y days. Ages range from (min #2) to (max #2). “Blah blah blah” text I enter later Congratuations! You have created your plan. Generate “output": Document summarizing their answers. They can save of download this. There is a section where they can add notes to this. This document will change if their answers change. that can be saved or downloaded and which will change if answers change. Include: Name of Plan Goal of Plan (#4) above List names and age of each person (Item #2 above) and room for more of my “blah blah blah” Generate “revenue": Allow me to put in text to offer them an upgrade to a paid version to develop a more detailed plan.
