Create simple 1d/eBook cover

I am looking for someone to create one professional looking 1dimensional report cover for me. Kindly read the following and let me know if you have any questions about the job. Here is a sample of what I will like: Please note the following: 1) I want the finished covers as a vertical rectangle-shaped and not square image. 2) I want the cover in two different sizes i) the first one 960x1280 ii) second one 600x800 and under 127KB in file size (they must both be saved in JPG images) 3) They must be royalty free. (I want to know where the images came from so that I can verify that they are 100% royalty free) 4) I will supply the textual content that should be placed on the cover- Author's name, Title etc 5) This is a work for hire, which means that I will own all the rights to the finished product and you must be willing to sign a contract stating this. This is something I reckon can be done by a skilled person in this area in a couple of minutes. I am offering not more than $4.00 for the job. If you are skilled in this craft, I reckon you will already have a template to work with, so it really should be a walk in the park for you.
