This can be absolutely implemented with the possible options like below; - SharePoint Add-in which offers a custom form built using jQuery + HTML + CSS. Form will allow user to record 3 time slots as expected. - SharePoint Single Page App (SPA) using Angular to record time slot booking as expected. Both of these options will store data to SharePoint lists and operate on them.
