* Will fix your all points one by one * Hope you are doing well. What I will do in these mentioned points, It will work fluent and perfect. 1) The form fields need descriptions next to each field. When a client adds a quantity they cannot see what they filled out. ----- For every fields of at quantity field 2) The +/- icons are too small and cannot be read. ----- which screen size you are testing? 3) The form overall is too wide. It needs to be more fitting for a page. ---- It will me more closer than a screen in responsive 4) The form results are not coming to my email address --- May be hosting issue or may be code edits 5) There is no thank you page for the form once it is submitted. --- Its a contact form 7 so need custom work 6) OPTIONAL: On the home page, eliminate the zip code and state field --- It will be done Thank you

1) it seems you are using contact form & plugin with some addon . Cf7 allows to add description to the fields. 2) i believe u r talking about the up and down arrows which are under inventory accordance 3) can u please tell how much you want to shrink the form like half page or so ? though i need to see how the inventory part is made and if it can be shrunk without damaging its functionality and also ensuring it looks nice. 4) are rest of the emails from site reaching your inbox? 5 its cf7 it usually display a thankyou msg rather than a page. but I think it got capability to redirect to a thankyou page after successful submittion. 6) if this form is being used on more than 1 place then its 1 form and just called on different places. removing 2 fields will malfunction for sure *** above you have mentioned that its not sending email. can you please clear this point? ******** SIR its not difficult but time taking playing with front end takes time. after 1 single change u have to refresh to see results.
