PHP MySQL HTML CSS JS Codeigniter Laravel Ecommerce Formbuilder RegEx

I am looking for a specialist to build me a web formbuilder that will be WYSIWYG application to drag-and-drop controls into a container with rows and columns with database connectivity to MySQL through PHP, HTML, CSS, JS and other web based technologies like laravel and codeigniter, to allow a form for data input creation, and then implement it into any type of web backend using MySQL database. There are formbuilder applications out on the Internet, but none do everything I'm looking for. I'm a disabled Veteran on a fixed income, so my budget is limited. I want to be able to format users input from US social security number, US telephone number, Canadian SIN, international telephones, credit cards, routing number, bank accounts, driver's licenses and any other specialized formats using RegEx for formatting/validation. I need to be able to size by pixel each contol to allow row or column setup based on form size, or container size on the reusable controls. See attached image.
