PHP Developer Needed - Employe Database

In the words of Axe from Dota, "I said Good day sir!" Hi everyone, my needs are simplistic. I need a development company to create an employee database management system for me. The spec is as follows: 1. Allow me to create employee records for all 500 employees in the company 2. We need to capture: Name, Surname, Date or Birth, Age, ID Number, Job Description, Manager, Race, Physical Address, Home Number, Cell Number, Personal Email, Postal Address, Spouse Name, Spouse Contact Number, Spouse Email Address, Qualifications, Drivers License, Medical Aid Name, Monthly Salary, Medical Conditions, Doctor's Name, Doctors Number, Work Start Date, Anniversary Date All this information will be text boxes, number boxes, calendar selectors or simple drop downs. This information (when saved) must appear in a nice neat grid with pagination. I also need this grid exported to Excel l and PDF. I also need to be able to click on a name and view all the information above. This must be exportable to Excel and PDF. All data in the grid must be searchable. I dont need reports. Its just for 1 user ONLY. No need for user levels. Just one level, Administrator. I just need this simple system only. PHP, MYSQL ONLY. A framework is preferred. MUST RUN ON CPANEL WEB SERVER (UBUNTU) I need to see samples of your PHP work. I want a very attractive modern design that looks excellent and has modern design trends and features like autocomplete search, modal popups, nice clean flat designs. Must also be responsive and run on mobile phones and tablets and laptops and desktops. I need to see screenshots of designs you have already done and also maybe send me links to demo systems you have completed.
