MySQL database development and web UI

I'm looking for a MySQL developer who can design/create a database as well as build a web user interface. There is a current database application built in an early version of FileMaker that will be replaced. Customer Information and a series of measures taken at customer residences are collected through a mobile forms application. Daily, users need to download data, csv or xls files from the mobile forms database, and upload the data directly into the database. There needs to be a relationship or lookup between the Customer Data and the Measurements Data. There are two sets of calculations that need to be applied to the Measurements Data: FIRST Calculations - Invoice data. Labor price, Material price, Support Material Price, Sales Tax and Total Cost. Based on the first set of calculations an invoice needs to be generated, available in csv format and printable in pdf format. SECOND Calculations - Payroll data. Labor price, Material Price, and Total cost. An invoice needs to be generated, available in csv format and printable in pdf format. This is a proof of concept project. If successful, I intend to add numerous other forms, data, and functionality which will require on-going development and support. Thank you.
