Hi Guys, Need a simple web page developed (PHP) and uploaded to a host in 1-2 days. It is an early registration page for our upcoming iPhone app. It just has two fields: username and e-mail address. When the user clicks submit, a confirmation e-mail is sent to them with a link (similar to most registration procedures), once they confirm, the username should be inserted to a MySQL table as reserved usernames. An e-mail containing info that they've successfully reserved the username should be generated and sent to them. All designs/graphics will come from us. That's it! I'm expecting to pay for 8-16 hours of work. Please include examples/screenshots of similar work you've done. No sample/demo/screenshot = no reply.
