Build a website landing page that performs an API call

Hello. When customer's scan my QR code I want it to take them to a simple landing page in theme of my company's main website. They input their address, the website performs an API call to a home valuation resource such as zillow and retrieves the estimated home value and square footage. The customer then receives a result in a $ value that is a fraction of a percent of their home value or a $/ft, whichever is less. Customer's can adjust a slider with an associated picture to indicate the approximate size/style of their home, for ex a single story range to a multi story complex architectural style that will apply a multiplier to their output $.
