I need a form with security logi and log out option (as attached) and upload to my hosting making it a password protected page. I need all field and some radio Button And a file upload option with select Button. File should be image and pdf extension. When user submit the form with all data it's should auto send me a email in my email. When i found the email i see one PDF with user all information and attachment file. When user click the submit Butto User Should See a Succes message as like your data SEND etc... Project Budget and Time lm : 20$ and 1 days. Thanks!
