JPG/ In Design File to Wordpress Theme

I hope you are all doing great. I need a WordPress theme designed. I will provide all the images/ graphics; also, I already have the designed created (In Design), all I need is help creating the code. I will worry about the main images and content; what I need is mostly to set up the structure and the points below. 1. DHL form (people will type the information in this form and the form will fill out the form in the DHL website and submit it) – Examples in attached image. 2. Plugins installed and fully modified. (The weather one is easy, just need to update the CSS, and the money one needs to be changed to also show Euros) – Examples in attached image. 3. Needs to design 3 pages (index page (attached image)- showing latest posts / single page - simple structure and same sidebar for the entire site / blog view page - small CSS changes to blog posts) 4. There is no need to get into details of kind of font, and most color ideas will be provided; this is mostly to build the structure of the theme. 5. The idea is to keep the costs low. I know which plug ins we will need for the weather and the money exchange rates. They will need CSS modifications, plus whatever is listed below. I have attached the image I have created that shows how the theme should look at the end. Once again, I will provide all the required images and graphics, I only need help with the structure and the changes with the plug ins. Please keep in mind that this is a low budget project.
