had to re-list this as the programmer I picked disappeared. I need a form for a competition where people have to fill in name, address, age, sex etc. in and also recommend friends by entering there name and email address. The more friends recommended the more entries the person will get in the competition. All the friends recommended will get an autoresponder. I need this and then I need a bit of graphics and HTML to make the 3 pages look good. 1. Entry form (name address phone) need a form with those fields: name address city postcode telephone no. 2. Additional questions (interests age sex recommend friends) age sex referred friends (20 fields, minimum 5 to be filled out) interests (I will send a list to the winning bid) username password 3.
