Code one page with some simple includes (signbase)(repost)

We have an existing PHP site with a MySQL database. We are changing the way the site works so you'll be working in a development directory. You will need to create some new database tables and then code one main "create" page which contains various includes to submit data to the database. There is also an "edit" page which needs coding - it's the same as the "create" page. Please read the attached document for full details and instructions. You will be given ftp and cpanel access.
