Add New User Page - PHP, MySql, Codeigniter, etc...

Here's what I need. This code is 98% done, I just need modifications to it. When an admin user adds a new member to the site, it is done through a form. Here are the changes I need to the form: 1. I need the form to generate a 4 digit random confirmation with a "BA" suffix added. (ex. BA-4D35). This code will automatically populate a text box when the form is loaded. 2. I need the code to automatically populate the "date created box" 3. I need the form to populate a few fields in a database that the form does not already populate. 4. I need the programmer to double check over the code that's already there to make sure there are no errors going in to the database. 5. Once whoever creates an account, I would like an email sent to the email address of the user, informing them an account has been created and also an email sent to a specified email address with the name, date created and the name of the person who created the account.
