PHP Form Needed to Update XML

I know this is a simple project as I almost got it right myself but I am not a PHP person. I have the files attached to work with. It is really just putting in the PHP component of the code and make it work right. What I need is a simple PHP form to enable updates of the existing data in that XML file and to clear all, as another option. The code will make it clear why I need. It must use simplexml so I understand it better later. I also need this done pretty much immediately so I can carry on. I got it to work with 1 entry but couldnt get multiples to work.
