input of stars rating in form

have html form and trying to put a 5 star rating system for products - go to [login to view URL] and click on "Reviews(1)" button to see the form I found code for a system to add but it is not working right - - I would like larger stars like it has but must be stable - current one the stars don't line up - the rating disappears if you hover over the form - etc. etc. also want to put "Rating: " or "Rate Product: " in front of the stars - but right now if I try it overlaps the stars - also want the line centered.
