HTML/CSS 2 pages [URGENT - must done in <3 hours]

I have 2 very simple pages (login) and a application form (just html input boxes and some javascript), I need you to redo the html/css so the form looks nicely on an existing page with header/footer (You will need to recreate the header/footer since they are on wordpress now and I want the page to be Off wordpress).
