Booking Forms for Website

Hi, I have a business [login to view URL] with side businesses [login to view URL] and [login to view URL] now what I wish for is the following: I run deals online as company campaigns.. I wish for a booking system set up that works like this: Must have a page for each of the following: Carpet Cleaning Booking Page Gardening Booking Page Mowing Booking Page Window Cleaning Booking Page Each slot must be different size so for example: Mowing time slots would be available from 7am to 4pm (Mon - Fri) and 8am - 1pm Saturdays Actual Mowing Slot Size will be 30 Minutes Actual Gardening Slot will be 90 Minutes Actual Carpet Cleaning Slot will be 90 Minutes Actual Window Cleaning Slot will be 60 Minutes For the carpet cleaning booking page I need the following settings: Mondays are Brisbane Southside Region Tuesdays are Brisbane Northiside Region Wednesdays are Brisbane East/West Side Region Thursdays are Goldcoast Region Fridays are Brisbane Southside Region Saturdays One Week (All Sides of Brisbane) Saturdays the following week (Gold Coast Region) The way the booking system must work is as follows.. Each side of brisbane/goldcoast is a different booking system. This booking system would ask the following questions before confirming booking: Name: Email: Address: Phone Number: Also before being able to confirm it will need another field just under these where they can upload a voucher. They MUST upload a voucher before they can proceed to the booking. Please note, that all Australian Public Holidays & as well as QUEENSLAND school holidays must NOT be available for bookings unless done MANUALLY by myself or my staff.. These dates cannot be booked online by customers. I am looking for something budget, as I can by these kinds of forms already made however I do require this little bit of customization
