I conduct employment polygraph exams. I use the same questionnaire and recieve different answers from numerous applicants per year. I need an application/questionnair turned into either an e-document or online application. I want the applicant to be able to type their responses (open ended response, not yes or no) to the question. I want them to be able to send the responses back to me and i want to be able to import the responses into an excel spreadsheet. I will use the imported data in a report (which i have already created).

Hi. Sounds good. So you need a form driven application which should record all the user input, posts into an admin panel where you review all the inputs and then export it into spreadsheet.
