Design small part of a webpage

Hello I want you to give me two quick mockups of HOW YOU would like this part to be, to get the visitor register for class just below this part. I want at least one image there, else is up to you. I just need some new ideas. I will give you 15USD for 1 hour work. Bids much higher will not be considered. Its a sellingpage for a dogschool, and im trying to sell my puppyclasses on this page. Target client is female between 35-55, married with kids and in fulltime work. Look at the attached 2 files before making a bid. If im happy with mockup i might hire again to make this better, and for several more pages. Thanks.
