Create a survey in php for me.

Hi everybody for the site of my club, I need someone to create a survey in php with Mysl database or not. Actually I would prefer with a xml file. - The survey will have 10 questions with 4 answers possible everytime. - When the survey is finished, a page will disply the result and the average results. - If possible in the result page, the listing of the participating persons. - All the questions have to be on the same page. Te col - If you have ideas to make this fun please propose. - The code will be copy/paste in the site so ask me for the adress to be as close as possible of the deign. I don't have very much money for this project because I do it for free, for the club. Regards, Christian
