Customer sign up, subscription payment and login/control panel

We require a customer purchase/sign up & payment form in HTML/CSS/PHP/MySQL for the 2 packages that we are offering. This form will be accessed from 1 of 2 purchase buttons depending on the service the customer is purchasing, it must check user name availability from the customer database before submission, collect customer business details & bank details, these will then be passed to a payment form which after collecting card payment details will start a weekly subscription payment through the Paypal web payments pro API. Upon completion this the customer will be sent a sign up confirmation email containing a user/password reminder, the details will be passed to a database with package information and POST package information to a script for automated package creation, the account/package will be marked as inactive until payment confirmation have been received through Paypal’s API at which point a account active email will be sent to the customer . We will require a password protected customer control panel/login area. The login page must have the option for lost password retrieval via registered email and to allow permanent login. The customer control panel will allow the customer to see payment history, change/edit their registered details (apart from username) and a payment form so they can process payments through our PayPal API. This then brings the requirement for a password protected admin area, the admin area will provide a summary of customers and allow editing of there details, activation/deactivation of customer accounts, allow setting of the 2 package prices and provide a record of all subscription payments received and other payments passed through the system by customers, we also need to be able to set processing rates/limits for customer transactions and have a summary page containing customer numbers, transaction numbers/amounts processed etc. We will also require a page header which will allow us to add more pages within this password protected this area. ? All the forms must have full validation for entered data and be written to prevent SQL injection attacks, all code must be fully commented in good English as this will form part of an on going project.
