Complete a form of code verification and add it to WordPress site as a plugin.

I have a form that needs completion, please read carefully and bid responsibly. NOW there's a form with a few ajax fields connected to mysql through 2 php files that send corresponding to those fields mysql queries. the form has submit button always enabled. SHOULD BE the submit button needs to be disabled until 2 php files generate successful responses for the form fields (if the form data is entered accurately and if it doesn't match the existing records in database - the condition code already exists) NOW After the form is submitted the user goes to another page for successful or not successful (depending on captcha result) response. The response is yet blank, except for the piece of code that generates random string. SHOULD BE If the recaptcha entered doesn't match, the user is redirected to the previous code (nothing happens) If the recaptcha matches, this random string should be sent in a get request to a dedicated http API interface (sms-gateway) and message ID should be returned back in a header and written to the MYSQL database and the user has to be asked in an opened box to enter the code he has just got in the sms-message (the random string that was generated previously). If the user enters this code correctly the data he entered should be saved into mysql db - card_table. If the user enters invalid code he is asked in the same box whether he wants to send another code or to follow the link to contact page for issue disputing (opens in the same window). SHOULD BE I need this form to be added to my wordpress template as a plugin. Situated on the right to the text 'Best parties ever!', preferably in unfolding <div>.
